- Background
The Government of Rwanda, through the Ministry of Health (MOH), is committed to strengthening maternal and newborn health outcomes by enhancing the quality, timeliness, and continuity of care provided across health facilities. Despite progress in reducing maternal mortality, preventable maternal deaths continue to occur due to delays in clinical decision-making, insufficient access to specialist support, and inconsistent use of real-time data for clinical management and planning.
To address these challenges, the MOH in collaboration of CIIC-HIN is implementing a project designed to institutionalize sustainable, data-driven mechanisms for improving maternal health care. The project integrates Quality-of-Care (QoC) indicators into national health information systems to facilitate the generation of timely, actionable maternal health indicators for decision-making at all levels.
A key component of this initiative is the deployment of virtual clinical support services, including VR-enabled and telemedicine tools, in selected hospitals. These technologies are aimed at enhancing rapid clinical decision-making, strengthening provider skills, and improving emergency response for high-risk pregnancies, complicated deliveries, and obstetric emergencies. The project will provide clinical support across key service areas, including ambulance services, emergency rooms, surgical theaters, delivery rooms, and intensive care units. The virtual support model aims to reduce the “second and third delays” in maternal care by ensuring rapid specialist input within defined response-time standards
To operationalize this virtual support system, a team of gynecologists and obstetricians will offer 24/7 specialist consultation to clinicians at 10 district hospitals. Given the need for coordinated scheduling, effective communication, consistent documentation, and harmonized clinical guidance, the project will engage the Team Lead-one Senior Gynecologist & Obstetrician to lead and coordinate the roster of specialists and provide technical expertise. This role is essential for ensuring uninterrupted coverage, maintaining quality and consistency in clinical advice, and supporting the overall effectiveness of the virtual clinical care model.
- Purpose of the position
- The Senior Gynecologist & Obstetrician will serve as the Team lead responsible for:
- Coordinating and managing the roster of all gynecologists and obstetricians involved in the project.
- Ensuring effective preparation, implementation, and monitoring of the duty roster.
- Providing senior-level clinical expertise to selected hospital teams during obstetric emergencies, high-risk deliveries, and complex C-sections.
- Supporting quality assurance, documentation, and reporting aligned with national and project standards.
- Strengthening communication and coordination between Gynecologists and Obstetricians and district hospitals teams.
- Establishing and enforcing standard response-time benchmarks (e.g., specialist response within 5–10 minutes for emergency calls)
- Ensuring medico-legal clarity in teleconsultations through standardized documentation templates
- Key responsibilities
3.1. Roster coordination and administration
- Develop and maintain a comprehensive duty roster ensuring 24/7 Gynecologists and Obstetricians coverage.
- Ensure clear communication of the roster to the pool of Gynecologists and Obstetricians, MOH/ clinical and public Health department, HIC, district hospitals, and CIIC-HIN project Lead.
- Oversee documentation and reporting of roster-related performance and challenges.
- develop a digital on-call tracking dashboard to monitor response times, case volume, and escalation trends
- Establish escalation protocols when primary on-call specialist is unreachable
3.2. Clinical leadership and technical support
- Supervise the provision of technical support to hospital team managing high-risk pregnancies, complicated labor, deliveries and C-sections.
- With other appointed specialists at HIC, offer real-time guidance through virtual platforms.
- Ensure that clinical advice aligns with national guidelines, WHO standards, and evidence-based practice.
- Support stabilization, decision-making, and timely referral of critical cases.
- Develop standardized virtual emergency management algorithms (PPH, eclampsia, sepsis, obstructed labor, etc.)
- Lead quarterly clinical case audits specifically reviewing virtual intervention effectiveness
- Ensure documentation of all teleconsultations in a structured format integrated into national HIC
3.3. Capacity building and mentorship
- Provide remote mentorship to district hospital teams including Junior Gynecologists and Obstetricians, GPs, and midwives.
- Support documentation of lessons learned for continuous quality improvement.
- Establish monthly virtual grand rounds for complex maternal cases across the 10 hospitals
- Develop simulation-based emergency drills using VR tools
3.4. Monitoring, Evaluation, and Reporting
- Ensure comprehensive documentation of all consultations supported by the pool of Gynecologists and Obstetricians.
- Consolidate monthly reports summarizing case types, response times, clinical outcomes, challenges and recommendations.
- Work with MOH, CIIC-HIN and other stakeholders to develop policies, SOPs and guidelines to improve workflow efficiency.
- Track key performance indicators (KPIs) including: Maternal near-miss rate, Emergency C-section decision-to-incision time, Referral appropriateness rate, Teleconsultation response time, Maternal mortality case review trends
- Produce quarterly analytical reports linking data intelligence findings to clinical outcomes
3.4. Coordination with project stakeholders
- Work closely with the MOH/HIC and CIIC-HIN project Lead.
- Participate in regular coordination meetings with district hospital’s leadership.
- Ensure strong communication between Gynecologists and Obstetricians and district hospital’s teams.
- Serve as clinical governance focal point for virtual maternal health services
- Coordinate with ambulance and EMS and ICU leadership to ensure pre-hospital teleconsultation integration
- Expected Deliverables
- A fully functional roster ensuring uninterrupted district hospital’s teams support.
- Monthly consolidated roster performance and clinical support reports.
- Documented improvements in case management and provider capacity.
- Contributions to updated emergency obstetric care guidelines and protocols.
- Evidence of strengthened coordination and communication across selected district hospitals.
- Reduction in avoidable referrals and improved maternal emergency stabilization rates
- Annual impact evaluation report demonstrating contribution to maternal mortality reduction
- Duration
The assignment will initially cover one year, with the possibility of renewal based on performance and project needs.
- Qualifications and Competencies
6.1. Essential qualifications
- Medical Doctor with a Specialist Degree in Obstetrics and Gynecology.
- Valid professional license registered with the Rwanda Medical and Dental Council.
- Minimum 7–10 years of progressive experience in managing high-risk obstetric care.
- Demonstrated leadership in coordinating medical teams or clinical services.
- Experience in handling obstetric emergencies, high-risk deliveries, and complicated C-sections.
- Prior experience in telemedicine, digital health systems, or remote supervision strongly preferred
6.2. Technical and administrative Skills
- Strong coordination, planning, and administrative skills.
- Proficiency in computer applications (MS Office, telemedicine tools, digital reporting platforms, etc…).
- Excellent analytical, documentation, and report-writing abilities.
- Ability to manage multiple priorities under time-sensitive conditions.

- Reporting
The Team Lead will report to the CIIC-HIN Project Lead and MOH/ clinical and Public Health Services department, HIC Focal Point, with regular interaction with hospital teams, HIC, and other implementing partners.
